# 17 Oct 2025: 📝 Form Builder – Create & Edit Your Own Forms

We’re excited to introduce the new **Form Builder** 📝✨

Organisations can now create and edit their own forms directly within the Clinical Portal — no development request required.

This unlocks greater flexibility, faster turnaround, and full control over your intake and operational workflows.

#### ✨ What’s New

### 🛠 Self-Service Form Creation

Under the Forms section, Organisations can now:

- ➕ Create new forms
- ✏️ Edit existing forms
- 💾 Save as Draft
- Publish when ready

No need to send forms for conversion — everything can be built directly in-platform.

### 📋 Flexible Form Structure

The Form Builder supports:

#### 🏷 Form-Level Settings

- Form Name
- Description
- Draft / Published state
- Option to allow patients to update their submission

---

#### 📚 Section-Based Layout

Forms can be structured into sections with:

- Section Heading
- Section Subheading
- Information Screens (rich text editor supported)
- Questions

---

#### ❓ Multiple Question Types

Organisations can configure different question formats, including:

- Free Text
- Multiple Choice (Single Select)
- Multiple Choice (Multi Select)
- Rating

Each question can include:

- Question text
- Helper text
