Adding Forms & Assessments to Bookable Services

Automatically send Forms and Assessments to patients when an appointment is booked 📅 or attended ✅ to save time and ensure patients have access to your desired Forms and Assessments at the most optimal time.


🔧 Where to Set This Up

  1. Head to Team Calendars → Booking Management → Bookable Services.

  2. Click + Service or select an existing service to edit.

  3. Scroll to:

    • Send Forms When This Service is Booked

    • Send Assessments When This Service is Booked

    • Send Forms When This Service is Attended

    • Send Assessments When This Service is Attended

  4. User the drop-downs to attach Forms and Assessments as desired.

⚙️ How It Works

📅 When the Appointment is Booked

  • Any selected Forms or Assessments will be automatically sent to the patient as soon as the appointment is booked.

  • Example: Send a Patient Registration Form or a CORE-10 assessment ahead of the session.

✅ When the Appointment is Marked as Attended

  • Forms and Assessments here will only be triggered once the appointment status is changed to Attended.

  • Example: Automatically send an Intake Form or a follow-up Outcome Measure after the session.


💡 Handy Tips

  • You can attach multiple items to each trigger.

  • Only active forms and assessments will appear in the dropdown lists.

  • This feature helps automate your workflow and ensures the right Forms and Assessments get to your patients at the right time 💼✨
