C-ACT

Children's Asthma Control Test

A short self-report measure of asthma control in children aged 4 to 11. It combines questions answered by the child with questions answered by a parent or caregiver, producing a single score that reflects how well the child's asthma has been controlled recently.


CAT-Q

Camouflaging Autistic Traits Questionnaire

A 25-item self-report measure of social camouflaging, meaning the strategies a person uses to hide or compensate for autistic traits in social situations. It reports three areas: compensation, masking and assimilation.


CORE-10

Clinical Outcomes in Routine Evaluation - 10 item

A 10-item self-report measure of general psychological distress. It covers anxiety, depression, trauma, physical problems, functioning and risk, and is designed as a brief outcome measure for routine, session-by-session use.

Copeland Symptom Checklist - Adult ADHD

A symptom checklist that helps identify ADHD in adults, developed by Edna Copeland. It covers eight areas, including inattention and distractibility, impulsivity, activity level, noncompliance, underachievement and disorganisation, emotional difficulties, peer relations and family relationships. It is a screening aid rather than a standalone diagnostic tool.

Depression, Anxiety and Stress Scales - 21 item

A 21-item self-report measure of negative emotional states, with three 7-item subscales for depression, anxiety and stress. It is the short form of the DASS-42 and measures severity over the previous week.


DASS-42

Depression, Anxiety and Stress Scales - 42 item

The full 42-item version of the Depression, Anxiety and Stress Scales, with 14 items each for depression, anxiety and stress. It measures the severity of these states over the previous week.


DERS-18

Difficulties in Emotion Regulation Scale - 18 item

An 18-item short form of the Difficulties in Emotion Regulation Scale. It assesses problems with recognising, accepting and managing emotions across six areas, including emotional awareness, clarity, acceptance, impulse control, goal-directed behaviour and access to regulation strategies.

DeJong Gierveld Loneliness Scale - 11 item

An 11-item self-report measure of loneliness that distinguishes emotional loneliness (the absence of close attachments) from social loneliness (the absence of a wider social network).


EDE-Q

Eating Disorder Examination Questionnaire

A 28-item self-report measure of eating disorder features over the previous 28 days. It reports four areas, restraint, eating concern, shape concern and weight concern, and asks about the frequency of key behaviours.

Flourishing Scale - 8 item

An 8-item self-report measure of psychological flourishing, covering a person's sense of purpose, positive relationships, engagement and self-worth. It produces a single overall wellbeing score.


GAD-7

Generalized Anxiety Disorder Seven

A 7-item self-report screening measure for generalised anxiety, asking about common anxiety symptoms over the previous two weeks. It is widely used both for screening and for tracking severity over time.


GBO

Goal-Based Outcomes Measure

A brief, personalised measure that tracks progress towards goals a patient sets at the start of care, with each goal rated on a 0 to 10 scale. It is widely used in child and youth mental health and complements symptom-based measures.


K10

Kessler Psychological Distress Scale - 10 item

A 10-item self-report measure of non-specific psychological distress over the previous four weeks. 


K5

Kessler Psychological Distress Scale - 5 item

A 5-item version of the Kessler distress scale, adapted for use with Aboriginal and Torres Strait Islander people. It serves as a brief indicator of psychological distress.


K6

Kessler Psychological Distress Scale - 6 item

A 6-item version of the Kessler distress scale, measuring non-specific psychological distress over the previous four weeks. It is a shorter alternative to the K10.

A 15-item self-report measure of dispositional mindfulness, meaning how often a person is attentive to and aware of the present moment in everyday life.

Obsessive-Compulsive Inventory - Revised

An 18-item self-report measure of obsessive-compulsive symptoms. It reports six areas: washing, checking, ordering, obsessing, hoarding and mental neutralising.

A 4-item outcome measure used to track general progress in care.


PCL-5

PTSD Checklist for DSM-5

A 20-item self-report measure of post-traumatic stress symptoms, mapped to the DSM-5 criteria for PTSD. It is used for screening, provisional assessment and tracking symptom change.


PHQ-9

Patient Health Questionnaire - 9

A 9-item self-report measure of depression symptoms over the previous two weeks, based on the DSM criteria for major depression. It is widely used for screening and for monitoring severity over time.


PSEQ

Pain Self-Efficacy Questionnaire

A 10-item self-report measure of a person's confidence in carrying out activities despite their pain. It is commonly used in chronic pain management to track functioning and treatment progress.


RAADS-R

Ritvo Autism Asperger Diagnostic Scale - Revised

An 80-item self-report screening measure for autism in adults. It covers social relatedness, circumscribed interests, language, and sensory and motor experiences, and is intended to support, not replace, clinical assessment.


RCADS

Revised Children's Anxiety and Depression Scale

A 47-item self-report measure for children and young people, covering anxiety and low mood. It reports six areas: separation anxiety, social phobia, generalised anxiety, panic, obsessive-compulsive symptoms and major depression.


RCADS-P

Revised Children's Anxiety and Depression Scale - Parent Version

The parent or caregiver report counterpart to the RCADS, covering the same six areas of anxiety and low mood in children and young people. It is often used alongside the child self-report version.

Rosenberg Self-Esteem Scale

A 10-item self-report measure of global self-esteem, a person's overall sense of self-worth. It is one of the most widely used self-esteem measures.

A 4-item end-of-session feedback measure of how a patient experienced a session, such as the relationship, focus and overall fit.

A short-form session feedback measure completed after a session. 


SIDAS

Suicidal Ideation Attributes Scale

A self-report measure of the severity of suicidal thoughts over the previous month. It assesses the frequency, controllability and intensity of such thoughts, their impact, and how much they interfere with daily life. It is intended for use as part of a broader risk assessment rather than on its own.

An 18-item self-report measure of eating behaviour, reporting three factors: cognitive restraint, uncontrolled eating and emotional eating.


WHODAS 2.0

World Health Organization Disability Assessment Schedule 2.0

A measure of functioning and disability developed by the World Health Organization, covering six domains: cognition, mobility, self-care, getting along with others, life activities and participation in society. It is available in a shorter 12-item version and a longer 36-item version.

World Health Organization Quality of Life - BREF

A 26-item self-report measure of quality of life developed by the World Health Organization. It covers four domains: physical health, psychological health, social relationships and environment.


WURS

Wender Utah Rating Scale for ADHD

A retrospective self-report measure in which adults rate their own childhood behaviour to help identify a history of ADHD. The full scale has 61 items, with a widely used 25-item short form.
