Assessments are Patient-Reported Outcome Measures (PROMs) β€” structured questionnaires designed to track a patient’s health, symptoms, or wellbeing over time.

  • Generate numerical scores

  • May include subscales (e.g., Depression, Anxiety, Stress)

  • Display trend graphs over time

  • Offer AI-generated summaries for clinical interpretation

  • Can be scheduled, manually requested, or auto-triggered (e.g., start/end of group program)

  • Can be manually requested, scheduled, or auto-triggered (e.g., start/end of group program)

Documents are static PDF files uploaded by Team Members and optionally shared with the patient. They are not interactive and cannot be filled in by patients.

  • Can be edited, hidden, or deleted
