Assigning and Managing Patients in Journeys
Once your Patient Journeys and Stages are configured, you can begin assigning patients to them. This allows you to track progress, trigger automations, and manage patient flow across different care phases.
This article covers how to assign patients, view their progress, and filter based on stage or status.
➕ Adding a Patient to a Journey
There are two ways to add a patient to a Journey:
✅ Method 1: From the Journey Itself

Go to Patient Journeys and select the Journey you want to assign the patient to.

Open the Patients tab.

Click + Add Patient.

Select a patient from the dropdown list.

Choose their starting stage.

Click Save.
✅ Method 2: From the Patient’s Profile

Go to the Patient List and open the patient’s profile.

Navigate to the Journeys tab within their profile.

Click + Add.

Select the Journey and starting Stage.

Click Save.
📌 This method is especially useful when working with an individual patient and wanting to quickly enrol them into a new Journey from their dashboard.
👁 Viewing Patients in a Journey
From the Patients tab inside a Journey, you can see:

A list of all patients currently or previously assigned

Their current stage and status:
-
Active – currently progressing through the journey
-
Completed – journey finished
-
Did Not Complete – patient exited the journey early

Their Journey start date
You can also access journey history from the Journeys tab within a patient's profile.
🔍 Filtering Patients
Use the filters at the top of the Patients tab to:

View by Journey Status (Active / Completed / Did Not Complete)

Filter by Stage to quickly see who’s in each part of the journey
This helps Team Members track workloads, monitor progress, and identify patients needing attention.
🔄 Moving Patients Between Stages

From either the Journey view or the patient’s profile > Journeys tab, click the three dots (•••) next to the journey.

Select Move Stage.

Choose the new stage.

Click Confirm.
✨ When a patient enters a new stage, any automations configured for that stage (e.g. sending forms or assigning tasks) will be triggered.
✅ Marking a Journey as Complete
When a patient completes the final stage of their care pathway:

Click the three dots (•••) next to the journey.

Select Complete.
The journey will remain visible in their profile, but will no longer be Active.
🚫 Marking as Did Not Complete
If a patient exits the journey early:

Click the three dots (•••) next to the journey.

Select Did Not Complete.
This allows you to track partial completions and disengagement while preserving patient history.
