CBT Thought Record

The CBT Thought Record is a structured Activity based on core Cognitive Behavioural Therapy principles. It helps patients identify and challenge unhelpful thoughts by working through a guided reflection process.


🧠 What It Includes

Each Thought Record walks the patient through a series of evidence-based steps:

  1. Describe the situation – What happened? When and where?

  2. Identify emotions – What did you feel? Rate the intensity.

  3. Automatic thoughts – What was going through your mind?

  4. Evidence for and against – What facts support or contradict this thought?

  5. Alternative thought – Is there a more balanced way to look at this?

  6. Re-rate emotions – How do you feel now?


📲 Patient Experience

  • Patients can complete Thought Records whenever they need to, from the mobile app or web portal.

  • Each entry is saved and can be revisited later.

  • Thought Records provide a structured space for self-reflection and cognitive restructuring.


👀 Team Member Access

  • Team Members can view all completed Thought Records in the Activities section of the patient profile.

  • Team Members can also post comments on Thought Records if desired.
