CBT Thought Record
The CBT Thought Record is a structured Activity based on core Cognitive Behavioural Therapy principles. It helps patients identify and challenge unhelpful thoughts by working through a guided reflection process.
🧠 What It Includes
Each Thought Record walks the patient through a series of evidence-based steps:
-
Describe the situation – What happened? When and where?
-
Identify emotions – What did you feel? Rate the intensity.
-
Automatic thoughts – What was going through your mind?
-
Evidence for and against – What facts support or contradict this thought?
-
Alternative thought – Is there a more balanced way to look at this?
-
Re-rate emotions – How do you feel now?
