CBTe Self-Monitoring Form
The Self-Monitoring Form is a flexible Activity used for tracking specific behaviours, thoughts, or patterns across the day. A common use is as a Food Diary within CBT-E (Enhanced Cognitive Behaviour Therapy), where patients log meals, thoughts, and associated emotions to build insight and support change.
🧠 What Patients Record
In a typical Food Diary (CBT-E) format, patients are prompted to log:

Time of eating

What was eaten or drunk

Where they were

What they were thinking or feeling before and after

Any urges, compensatory behaviours, or notable events
📲 Patient Experience

Patients complete entries throughout the day in the mobile app.

The form is intentionally simple and quick to fill out, promoting consistency.

Team Members can view all entries via the Activities section of the patient profile.

Responses are presented in a structured format to help identify patterns and support discussion in therapy sessions.
