My Dashboard Overview

The My Dashboard tab is your central hub for managing day-to-day responsibilities as a Team Member in Wellifiy. It provides a real-time snapshot of patient engagement, upcoming appointments, tasks, assessments, activities, and more — all in one view.


🗓 Upcoming Appointments

This section lists Upcoming Appointments scheduled in the coming days. You’ll see:

  • Date & Time

  • Type (e.g. Group or Individual)

  • Service

  • Patient Name & Phone (if available)

  • Duration

  • Action Button () to join video calls or manage the appointment

Click Go To Calendar → to access your full calendar view.


✅ My To-Do List

Track outstanding tasks that have been assigned to you or created by you. Each task shows:

  • Task Name (e.g. Send program information, Medication review required)

  • Patient Name (clickable to open their profile)

  • Due Date

  • Status (e.g. Overdue)

  • Creator & Assignee

You can:

  • Filter by Team Member

  • Filter by task creator

  • Filter by status (e.g. Current, Completed)

  • Click + New to create a task

  • Mark tasks as complete via the checkbox


👥 Last Active Feed

Displays the most recent time a patient was active in the platform, including:

  • Patient Name

  • Last Active Date & Time

Use this to monitor recent patient engagement.


📄 Assessments & Forms Feed

Lists recent patient submissions, including:

  • Assessment or Form Type

  • Patient Name

  • Date & Time Submitted

Click on the highlighted link to view the full submission.

Note: This feed will display patients for whom the logged-in Team Member is the Primary Team Member, or where the Team Member has opted to receive notifications.


📌 Activities Feed

Tracks patient submissions of activities. Each entry includes:

  • Patient Name

  • Activity Type 

  • Date & Time

Click any entry for further details.

Note: Like the Assessments & Forms feed, this section only includes patients for whom you are the Primary Team Member, or where you've elected to receive notifications.


👩‍⚕️ My Patients List

This table shows all patients for whom you are listed as the Primary Team Member. For each patient, you’ll see:

  • Patient Name

  • MRN

  • Assessments: Pending and Completed

  • Forms: Pending and Completed

  • Group: Patient Group assignment

  • Labels: Clinic-defined tags (e.g. Engaged, Follow Up Required)

  • Date Created: When the patient profile was created

Includes:

  • A Search bar to filter by name or MRN

  • Filter options by Group and Label

  • + New button to create a new patient (depending on permissions)
