Patient Journeys: Overview
Patient Journeys are structured care pathways designed to help your clinic deliver consistent, stage-based support for patients across the full continuum of care.
Each journey is built around a series of configurable stages, and at every stage, you can set up automations, such as sending forms and assessments, assigning internal tasks, or sending messages. This allows teams to streamline repetitive processes, reduce manual admin, and ensure nothing is missed.
By using Patient Journeys, your team can optimise clinical workflows, deliver a more consistent experience for patients, and maintain visibility across each patient’s progression through care.
🔍 What Is a Patient Journey?
A Patient Journey is a configurable sequence of Stages that reflect the phases of care a patient goes through. For example, an “Addiction Recovery” journey might include:

You can customise each Journey to suit the clinical pathway relevant to your organisation or program.
🧩 What Are Stages?
Each Journey is made up of Stages. These are distinct points in the care process, and each one can include:

Stages help ensure the right actions are taken at the right time, and allow your team to automate parts of the workflow.
👥 Patients and Journeys
Patients are assigned to a Patient Journey and will progress through the configured stages. From the Patients tab, you can:

View which stage each patient is currently in

Filter by status (Active, Completed, Did Not Complete)

Add new patients to the journey

Track journey progress at a glance
Each patient's current stage determines what automations are triggered (e.g. forms sent or tasks created).
